Secure Filing of Your Documents

On this page, we offer you the possibility of sending us your documents securely via our secure document filing platform.

Estate Documents


We’ll soon be directing you to the documents you need in your particular situation, based on additional questions we will ask you.

IMPORTANT: Please wait to speak to one of the notaries of Proactio or to receive instructions from us before ordering documents that may cost you money.

We’ll be in touch soon, so don’t be discouraged!

You can, however, send us any estate documents you may already have in your possession. Here are the documents you can send us, if applicable:

  • Copy of the act of death from the Registrar of Civil Status;
  • Will search certificate from the Chambre des notaires du Québec;
  • Will search certificate from the Quebec Bar;
  • Complete will (notarized, witnessed or holograph);
  • Codicil or testamentary modification (notarized, witnessed or holograph);
  • Judgment of probate or minutes of probate by notary (for witnessed or holograph wills);
  • Marriage contract;
  • Declaration of heredity;
  • Deed of appointment of liquidator.

Certain particularities may apply to your estate file. Additional documents may be required in addition to those listed. If so, we will contact you.

Document Proving Diagnosis Not Required


We remind you that information concerning the diagnoses of members of the CQTS-Blais class action will be obtained by the lawyers from the Quebec Cancer Registry or MED-ECHO. It is only if the diagnosis cannot be confirmed that proof of diagnosis will be requested.

If you already have medical records or a doctor’s note, you can send us these documents. Please note that these documents will not be analyzed at this time.

Proof of Cigarette Consumption by Declaration


Please do not send us invoices for cigarette purchases or photos of cigarettes or someone smoking. We will delete any such documents received.

You will be able to prove the date you started smoking and the number of cigarettes you smoked by affidavit.

We’ll have commissioners of oaths who can swear you in to sign the claim form when it’s time to claim.

You will receive an email from us when we begin the process.
