You must sign up with us if you wish to benefit from our assistance in the preparation of your claim file.
Before continuing to the registration form, please make sure you have the health insurance number of the tobacco victim. This information is essential to register online.
If you do not have the health insurance number of the tobacco victim in hand, please call us at 1-888-880-1844 so that a member of our team can register you.
Only one file can be opened per tobacco victim. In the case of an estate, a single person will be responsible for providing all information related to the estate and the heirs.
Important: The registration form is not a claim form. Registration does not create any right to compensation. With the information provided in the registration, we will be able to prepare your claim file. We will take care of submitting your claim file to the administrator once it has been finalized and signed. It will then be up to the administrator to determine your eligibility for compensation.

Upcoming steps in your file following your registration
1. Examination of your file: We will examine your file to ensure that it is complete.
If any information or document is missing, we will contact you by email or phone to obtain them.
2. Diagnosis Confirmation: Using the diagnostic information from the Québec Cancer Registry, MED‑ECHO, and the RAMQ, we will confirm the diagnosis in your file, if possible.
If we are unable to confirm the diagnosis, you will need to provide proof of diagnosis. We will then guide you to the appropriate resources to obtain the relevant medical documents.
3. Review: Once your claim file is ready, we will send you a draft by email so you can review it. You will need to verify that the content is accurate and complete.
No changes to the claim file can be made during the signing appointment. All modifications must be made beforehand.
If the content of the claim file is accurate and complete, you will be able to book an appointment to sign the form in front of one of our commissioners for oaths. Instructions for booking the appointment will be included in the email containing your claim file for review.
4. Swearing-in: You will have a videoconference meeting with one of our commissioners for oaths to sign your claim form.
5. Submission to the administrator: We will take care of the final submission of your claim file to the administrator, Epiq.
6. Administrator’s decision: Once the file has been submitted, you will have to wait for the claims administrator’s decision to find out if your claim has been accepted.
There is no delay specified for the claims administrator to render its decision in the Quebec Administration Plan.
